Prescription Drug Plan Maintenance Drug Request
Use this form to request that Sun Life add a specific drug to your pharmacy access, so you may receive reimbursement for up to a 100-day supply of a particular maintenance drug instead of the usual 34-day supply.

Notice to Residents of Manitoba |
The Manitoba retail sales tax of 7% will be added to premiums paid for insurance plans (Basic and Optional Life Insurance and Accidental Death & Dismemberment). This change will take effect for premiums paid on or after August 1, 2012 and will be deducted from your Monthly Pension Payment (DB members) or deducted from your bank account (DC members).
